CRISIS INTERVENTION AND SPECIALIZED
SHORT-TERM SERVICES
24 hour Telephone and Mobile Crisis Intervention:
Telephone and mobile (in-person support is available within the City of Sarnia) crisis response is available 24 hours per day, 365 days a year (including holidays) to anyone who may be suffering from mental illness, anyone who is in crisis related to a mental health problem, or anyone in the community who is experiencing thoughts of suicide.

Short-term Services:
Together with a Community Nurse or Social Worker, individuals with moderate mental illness, including Anxiety Disorders and Depression, will work on symptom management, health education, and crisis/relapse prevention planning, on a short-term basis. Services may also include assistance in engaging physicians in the treatment of moderate mental illness.
Discharge Planning:
Discharge Planning services are available for all referred patients from the Mental Health Unit at Bluewater Health.
Court Diversion Services:
Support is provided to individuals who have a mental illness and are involved with the Justice System. When indicated, the CMHA Worker can assist by advocating for court diversion.
Release from Custody:
In-reach services and follow-up care are provided to individuals in secure custody. Support, and the coordination of mental health follow-up care, is provided to integrate the individual back into the community.
Crisis Safe Beds and Emergency Housing:
Immediate support and outreach is provided to individuals suffering from mental health issues who are in need of emergency, or stable, housing.
Dual Diagnosis Nurse Specialist:
Nursing assessment and support is provided to dually diagnosed individuals (individuals with a mental illness and a diagnosis of developmental delay). The Nurse works with individuals, their families and other service providers to develop crisis prevention plans, facilitate community integration and coordinate developmental and mental health services.
